The Superior Court in Pima County website has Civil Court forms that include detailed instructions and information for filing an application for name change for an adult, minor, and/or adult(s) with children. These forms and instructions include information about walk-in, same-day name change hearings, filing by mail, and other helpful information. The Superior Court in Pima County website also has forms for requesting a fee deferral or waiver.
You can file your paperwork in person any day of the week, but walk-in same-day name change hearings are only conducted on Wednesday and Thursday mornings. If you want to be seen the same day you file your packet, please arrive at the Courthouse on a Wednesday or Thursday morning 30 minutes prior to the scheduled hearing times. Wednesday hearings are held at 10:00 a.m., and Thursday hearings are held at 9:00 a.m. and 10:00 a.m.
If a name change order is granted and shows that the Office of Vital Records is ordered to amend the birth record to reflect the new name, then you must provide a certified copy of the name change order to the Office of Vital Records, which may have additional requirements and fees.
