FILING A NAME CHANGE

1)  Obtain the forms from a bookstore, stationery store, chain pharmacy, or office supply store, or online on this website under On-Line Forms.

2)  When ready to file, bring the completed forms to room 131A at 110 W. Congress.

     Please arrive at 9:30 AM any working day to allow time to establish the case before the hearing.

3)  Present the paperwork, fill out a cover sheet (two different ones if attending court that same day) and pay the filing fee which can be found online on this website under Filing Fees.
